Referrals & Intake
HIV positive individuals (or those seeking an HIV test) can refer themselves to the JRC services. A physician referral is not required. Referrals can also be made by general practitioners, specialists, public health nurses, AIDS service organizations (ASOs) and other community-based organizations.
A social worker will do an intake interview (by phone or in person). This includes a brief assessment and orientation to available services including other HIV services in the community. Once an individual has decided to seek care in the JRC, the social worker will arrange an appointment with a nurse - Nursing 1st Contact. At this visit the nurse discusses medical history, facilitates baseline lab work and consent forms are reviewed and signed. Patients will receive an orientation to the clinic, clinic’s policies, hours and services. The nurse will set up a subsequent appointment with one of the JRC family physicians.
