Referrals & Intake
HIV positive individuals (or those seeking an HIV test) can refer themselves to the IDC services. A physician referral is not required. Referrals can also be made by general practitioners, specialists, public health nurses, AIDS service organizations (ASOs) and other community-based organizations.
A social worker will do an intake interview (by phone or in person). This includes a brief assessment and orientation to available services including other HIV services in the community. Once an individual has decided to seek care in the IDC, the social worker will arrange an appointment with a nurse. The nurse discusses medical history and facilitates necessary lab work. Patients receive an orientation to the clinic’s policies, hours and services. The nurse will set up a subsequent appointment with one of the IDC family physicians.
